Measles is a highly contagious respiratory virus that can produce severe health effects. The virus is spread through airborne droplets and can remain on surfaces for up to 2 hours. Symptoms of measles include fever, cough, runny nose, red watery eyes, and a red blotchy rash. Up to date information on measles exposure sites in Ontario can be found online through Public Health Ontario.
In the event of a measles exposure at the camp, Public Health will ask any children and staff who are not fully protected with two documented doses of measles vaccine to remain home for 21 days from the date of exposure or until evidence of measles immunity is provided.
Please acknowledge that you have read and understood the following at the bottom of this form.
- I acknowledge the ongoing public safety concerns related to the resurgence of measles in Ontario, and agree to cooperate with measures necessitated by the Public Health unit in my region:
- Waterloo/Cambridge: Region of Waterloo Public Health
- Grand Bend: Lambton Public Health
- Penetanguishene: Simcoe Muskoka District Health Unit
- I attest that my child(ren) is/are not exhibiting symptoms of measles (see above) that cannot already be attributed to a known, non-contagious source, nor have they been in close contact with a known or suspected case of measles prior to participating in camp.
- I agree to monitor my child(ren)’s health daily and will not send them to camp if they are showing signs of illness or are awaiting results of any relevant communication from my healthcare provider or Public Health.
- I understand that I have the opportunity to provide Public Health with the measles vaccination dates for my child(ren), which may help Public Health confirm their immunization status more quickly, and decrease the potential for my child(ren) to miss time at camp.
- I understand that the Drayton Entertainment Youth Academy will be cooperating with Public Health, and will be providing information to them upon request.
- I acknowledge that the Drayton Entertainment Youth Academy will be following Public Health’s guidance in determining if there has been an exposure at camp, as well as communication surrounding potential or confirmed exposure with families and other relevant parties.
- I agree to arrange pickup of my child(ren) in a timely manner should any concern arise regarding their health during the camp day. I further agree to keep my child(ren) home if they are ill.
- I understand that if my child(ren) is/are identified as unvaccinated or potentially exposed, they may be required to remain home for the period outlined by Public Health, even if asymptomatic.
- I understand the Drayton Entertainment Youth Academy’s existing refund policies still apply, and that exclusions or lost program time due to following the Region of Waterloo Public Health’s direction is outside of the Drayton Entertainment Youth Academy’s control and may not be grounds for a refund or credit.
